INTRODUCTION

Every child is an individual who has his/her own rate of physical development and own pace of learning. Each child is accepted, loved, nurtured and taught as an individual with this thought in mind. I encourage and build each child’s imagination and creativity. Instill a greater sense of self worth and encourage self-confidence.

I provide activities and an environment that will nurture the child’s self esteem. I do my best to have the child gain and keep a positive self-image. My main concern and goal in caring for your child is to see your child grow physically, intellectually, socially, and emotionally, to the best of his/her ability.

I will provide clean and well-supervised play areas.

Your child will be cared for with love, learning activities, positive discipline and lots of fun.

ADMISSIONS

Admission is available to any child between O and 3 years old. My Second Home offers an optional English/Spanish program where children will be spoken to and taught in both languages.

FEES AND TUITION

$50.00 non-refundable registration fee is required.

Payments are on Mondays with no exceptions.

Cash, checks, money orders or company vouchers are accepted. However, if a check or voucher is returned unpaid for any reason, you will owe me a service charge of $30.oo in addition to the amount due, and there after, you must pay by money order.


Full Time: O-2 years of age……. 250.00/week
Full Time: 2-3 years of age …… 250.00/week
Before OR after school .............. 100.00/week
Before AND after school ........... 120.00/week

OPERATIONAL HOURS

My Second Home is open from 7:00 a.m. to 6:00 p.m.

Children may be dropped off or picked up at any time during the specified hours.

Please advise the Child Care prior to 8:00 am if a child will be late or absent for any reason.

All children must be picked up by 6:00 p.m. there will be a fee of $5.00 for every 5 minutes after 6:15 p.m if the child is not picked up.

VACATION AND HOLIDAYS

My Second Home will be closed for two weeks each year for which you will pay two weeks. At least one month advance notice will be given. If the child is absent for any reason, payment is still required to retain the child’s enrollment and space in My Second Home.

The following days are considered paid holidays:


- New Year's Day
- Martin Luther King's Birthday
- President's Day
- Memorial day
- Independence Day
- Labor Day
- Columbus Day
- Veteran's Day
- Thanksgiving Day and the day after
- Christmas Day

I will have three days of sick leave in case I become too ill to provide care.

MEALS

My Second Home will serve one breakfast, two snacks, and a lunch (optional). All mealtimes will meet the nutritional and quantity guidelines set forth by the U.S.D.A. Nutrition Guidelines.

ILLNESS

Any days that I am ill and not able to care for your child, parents will be notified ASAP and arrangements will be made by parents for alternate day care.

If the child is sick, the provider is not responsible of taking care.

If while in care a child shows signs of illness, parents will be notified immediately and arrangements must be made to pick up the child. Signs of illness are but not limited to: cold, fever, diarrhea, rash, vomiting, pink eye, other contagious symptoms or any requiring special attention.

If child is thought to have a communicable disease, parents will be notified and asked to pick him/her up no later than one hour after being contacted.

DISCIPLINE

Physical punishment will never be used. Discipline is done on an individual basis. If a child does not listen, talks back, etc, the child will be asked to sit and think about it for one to five minutes depending on the child’s age.

Any discipline teaching always will be on the bases of mutual respect to the others.

TRANSPORTATION POLICY

Permission for the provider to transport is granted by a separate form. Car seats or booster seats are required by law.

Our mutual responsibilities:
- Treat each other and your child with respect.
- Communicate on a regular basis about the child’s rest, nutrition, toileting, new skills, interests and joys.

SUPPLIES

Parents are responsible for supplying the following:
Infants: Diapers, wipes, two changes of clothes, formula, bottles and baby food. If disposable bottles are used, parents responsi- ble for providing liners.
Toddlers: Diapers or training pants, wipes, two-three changes of Clothes to include socks, depending on potty training achieve – ments.
Preschoolers: Change of clothing to include socks.

TERMINATION

Either parent or provider may terminate childcare after two weeks written notification. I expect to receive payment for those two weeks.

EFFECTIVE DATE OF AGREEMENT

This agreement will take effect on __________________ and remain in effect until a change is made in writing (with two weeks written notice), or until ______________________.

At which time it will be reviewed and updated as needed.

A trial paid period is agreed upon. It will last for_____ weeks

Either parent or provider may withdraw from the agreement to provide care during this specified time.
